LICENSED PRACTICAL NURSE FULL TIME NIGHT SHIFT RN – LAFAYETTE, LA

Lafayette Physical Rehabilitation Hospital (LPRH) seeks a Full Time Night Shift Licensed Practical Nurse, LPN to provide direct and indirect patient care services that meet the psychosocial, physical and general aspects of care; meets the communication needs of patient and family; provides care that reflects initiative and responsibility indicative of professional expectations, under the supervision of a Registered Nurse.  Maintain regulatory agency requirements, nursing and hospital policies, procedures and standards.  Communicate with physicians and team members about changes in patient’s clinical condition, including results of diagnostic studies and symptomatology. Respond quickly and accurately to changes in condition or response to treatment.  Additionally, is able to perform general nursing duties in all departments with adequate supervision. Shift Differentials are paid for Night and Weekend Shifts!

CLINICAL LIAISON WITH PHYSICAL REHAB EXPERIENCE – LAFAYETTE, LA

Lafayette Physical Rehabilitation Hospital seeks a Clinical Liaison responsible for working directly with the Hospital Development Team, including but not limited to other Liaisons, Administrators, and Admission Coordinators. A Clinical Liaison is responsible for meeting with, communicating and providing education and support to the medical community. There are two basic parts to the CL role: evaluate the patient and educate the medical community. Clinical experience is required - preferably an experienced RN, LPN or PT!

REHAB REGISTERED NURSE FULL TIME NIGHT SHIFT RN – LAFAYETTE, LA

Lafayette Physical Rehabilitation Hospital (LPRH) seeks Full Time Night Shift Rehab Registered Nurses, RN to provide general nursing care to patients in the hospital by completing a psychosocial and physical, environmental, self-care, educational and discharge planning assessment of the patient through interview, observation and inspection, auscultation, palpitation and documents on all admissions per service line policy; Responsible for identifying patient’s present and potential problems from information gathered in the initial and continued assessments; Collaborates with physicians, interdisciplinary team, client and family in making decisions regarding patient’s need for nursing care; Initiates problem list within time frame established by service standard using approved taxonomy; Prioritizes problems based on client’s health status; Establishes specific and measurable individualized client care goals; Discharge needs/plan reflective of interdisciplinary approach and are initiated on admission. Shift Differentials are paid for Night and Weekend Shifts!
